If you have it, enter the model name or number of the item, e.g. FTDX-10
If your item has a date of manufacture stamped on it (rare), include it here. If it just has the month/year, make it the first of the month. If it has year only, make it January 1 of that year.
How obtained
How did you come into possession of this piece of gear?
Purchase date or otherwise when obtained
From an individual? Hamfest? Store? You can include their contact details also, just in case you need to reach back out.
We recommend you just include the net price without the shipping or tax. This will help you to later either compare or resell the gear. Of course, it’s up to you!
